Some families eventually consider assisted living, memory care or skilled nursing placement.
Horizon Care Services can help families understand whether additional private-duty home care may be available. Families seeking senior living placement may also be referred to Elder Care Finders, a separate company specializing in senior living and care-community placement.
Facility recommendations, admission requirements, availability, pricing and payment options must be verified with the placement professional and selected community.
