Assisted Living Placement

When living at home is not a good option.

When a person needs regular assistance with the activities of daily living (ADLs) or when being discharged from the hospital, an assisted living facility may be the preferred choice. Horizon Care Services can assist you in finding an appropriate licensed Florida assisted living facility when needed. Our care managers can provide a free personalized service to help you find the right place. This is an important decision; let us help you.

Choosing the proper assisted living facility can be a challenge for anyone but especially for an elderly person or a family member who does not live in the local area. Our care managers at Horizon Care Services are familiar with the facilities in the area and know their reputations. We can provide placement assistance for senior individuals who are seeking alternative housing throughout South Florida.

Our care managers provide a personalized service by first assessing the needs of our clients. Health concerns such as Alzheimer’s disease, Dementia, Parkinson’s disease, stroke, diabetes, and heart disease are addressed when referring to an appropriate facility. We will even drive you to the facility and assist you in communicating with the facility staff if this is needed.

An assisted living facility provides assistance with the ADLs, which may include eating, dressing, bathing, grooming, toileting, and transferring (moving between surfaces such as the bed, chair, and bathtub or shower). They also provide medication management and transportation to appointments and outings. A good assisted living facility will also have a variety of planned activities to promote healthy social interaction and mental stimulation.

Family members are encouraged to be part of the process in selecting an appropriate assisted living facility. They are also encouraged to be involved in future decisions and communicating with the facility staff and the staff at Horizon Care Services.

Some assisted living facilities, but not all, provide a secure environment for patients with Alzheimer’s disease or other forms of dementia. This security prevents residents from wondering off and becoming disoriented and lost. Horizon’s care managers can assist you in finding the facility that offers the level of services that you may need.

When providing assisted living placement services we review each facility and follow strict criteria. We look for the following before referring our clients to a facility:

  • Florida licensure
  • Cleanliness and order
  • Quality and friendly staff and caregivers
  • Nutritious meals served to meet the needs of seniors
  • Activities, fitness programs, and outings
  • Safety and security systems
  • A good reputation
