Our focus at Horizon Care Services has always been for seniors to stay at home safely for as long as possible. However when health problems are a concern, illnesses develops, or accidents happen, an elderly person may need additional home care services or care in a facility. Our goal as care managers is to coordinate needed healthcare and community services to meet the short-term and long-term care of our elder clients and their families. Without proper planning and coordination of homecare, unnecessary expenses and institutionalization may occur.
Our care managers assist elderly and disabled individuals and their families with short-term and long-term home care needs. We strive to empower seniors and their families. Whether family members live near or far, they can be assured that their elderly mother or father is personally respected and cared for properly.
