Horizon Care Services is a licensed Nurse Registry and is always looking for RNs, LPNs, CNAs and HHAs to work private duty cases. We have offices located in South Florida. We currently service Miami-Dade, Broward, Palm Beach, Martin, Lee and Collier counties. • Liability Insurance (If you have a current policy)
  
• Certificate from School stating educational hours (CNA and HHA only)
  
• Original 4hr HIV
  
• Assistance with Medication or Medication Management (CNA and HHA only)
  
• Alzheimer/Dementia Certificate
  
• HIV Update
  
• OSHA Update
  
• Domestic Violence Update
  
• Medical Errors Update (LPN and RN only)
  
• Physical (completed within 6 months and MUST state Free of Communicable Disease.) We do not accept physicals provided by Chiropractors
  
• Chest X-Ray or PPD
